Payments
We only accept payments in Cash or by Cheque and payments by Cheque are to be made
payable to ‘Sheffield Fibromyalgia Self Help Group’ and forwarded onto our Treasurer
‘Joan Leesley’ at 20 Smithy Carr Avenue, Burncross, Sheffield. S35 2ZQ.
We only provide receipts for payments on request.

Prices and Advertising Offers
Prices exclude VAT, postage and packaging costs, unless otherwise stated. All offerings
are subject to errors, omissions, change of prices or withdrawal of offers without notice.

Membership and Fees
Group membership fees cover a period of 12 months from the date the first payment is
received and are renewable after that period. They are subject to change at any time due
to inflation and costs incurred by the Sheffield FMS Self Help Group.

Donations
All donations are accepted on our approval and on a non-refundable basis.

Cancellations
Notification of any cancellation of membership or purchase of goods must be received
within 7 days of purchase.

Refunds
Refunds for payments made to FMA UK must be applied for by contacting FMA UK’s
Head Office: The Fibromyalgia Association UK (FMA UK), PO Box 206, Stourbridge,
West Midlands. DY9 8YL and are subject to their own terms and conditions.
Refunds for payments made to Sheffield FMS SHG are subject to the approval of the
committee members.

Reciprocal Links
Sheffield Fibromyalgia Self Help Group as the sole rights to include or exclude any link
that does not conform or abide to UK Laws on advertising. We will remove any link if it is
found that our own link has been removed or not working on our partners website. We will
not accept any links that promote or sell products associated with violence, pornography,
crime, drugs or any other illegal act.
